A depression where the grain shows

We just had our oak hardwood floor refinished and are not happy with the finish. It looks like the polyurethane is uneven. You can especially notice it when squatting down and looking forward. Wherever the grain of the wood is showing it looks like there is a bit of a depression.

Nail the new floor over the old floor?

I have 2 1/4” wide birch hardwood floor and I would like to replace it with 3 1/4” oak hardwood. Can I nail the new floor over the old floor or would I have to remove it and nail down a 3/8” plywood sub floor?

Bubbles of polyurethane along the cracks

Our new stained oak floors have developed bubbles of polyurethane along the cracks between the boards that flake off.

Grooves not cleaned before finishing

My hardwood floor was refinished with something called Rejuvenate. Unfortunately, the grooves between the slats were not cleaned and look dirty. What can I do or use?
