Hardwood finish is peeling after 6 months

Approximately 6 months ago I had some hardwood flooring installed. I now see some peeling going on. What should I do?
We had wood flooring installed in several rooms in late May – early June 2007. The carpet and tile were removed and the surfaces prepared by the installer, glue applied and the planks installed. After a few months and now more time, we experience popping noises throughout the house as if the glue under the plank is loose. Can the floor be repaired without removing all the planks? Does glue need to be reapplied? What is the suggested solution with minimal disruption to the existing flooring?
I just built a new stair case. Is there any thing I can do besides glueing the treads down? (In case I would ever want to remove them.)
My pneumatic hardwood floor nailer sometimes leaves unsightly marks my prefinished hardwood when I am nailing it. Is there something I should put over the plate on the nailer that will prevent it from marking the hardwood?
Our maple floors have been sanded to the bare wood (light). The stain was applied by a professional. There are dark streak marks, hand prints, spots and splotches over sections of the floor. Continue to stain or sand again and put clear varnish?
